    Works under the supervision of a Registered Nurse to provide nursing care to an assigned group of patients based on the established care plan.  The plan is implemented following established standards of nursing and the policies and operating instructions of Memorial Healthcare Center.  She/he may function in the capacity of a team leader under the direction of the charge nurse, or as a team member under the direction of a registered nurse team leader. 






    Graduate from an accredited school of practical nursing; completion of an accredited

    pharmacology course.  LPN license from Michigan or a permit to practice.



    Related experience preferred, not required.  BLS for Healthcare Providers is required.




     1.    On admission, assists with initial data and development of care plan in conjunction with RN.


     2.    Completes daily assessment of physical, psychological, and social dimensions and the educational needs of the patient and communicates significant findings to the supervising RN.


     3.    Modifies the care plan based on identified patient needs.


     4.    Implements and documents effectiveness of plan of care in the medical record.


     5.    Recognizes changes in status of the patient, provides appropriate intervention and/or communication to supervising RN regarding the need for further intervention or referral.


     6.    Provides ongoing patient and family education based on daily assessment of disease process, care requirements, and expected recovery.


     7.    Participates in inservice and/or continuing education to maintain and improve understanding of the present and future trends in health care, and participates in unit and hospital wide committees to identify needs and assist in continuous performance improvement.

    8.          Demonstrates job related aspects of patient safety to assure a safe environment, safe and effective use of equipment and technology, and decreased risk of potential adverse patient occurrences.


    9.       Demonstrates knowledge of and supports hospital mission, vision, value statements, standards, policies and procedures, operating instructions, confidentiality statements, corporate compliance plan, customer service standards, and the code of ethical behavior.
