We are looking for a Case Manager RN to join our interdisciplinary team (IDT) providing health care services and treatment for the PACE program participants.  In this position, you are responsible for assessing, coordinating, monitoring, and providing high-quality care, including coordination with the primary care team.  This is a safety sensitive position.  The position includes:



  • Participate in initial, semi-annual, unscheduled, and annual assessments; develops care plans based on assessments and updates on an ongoing basis, attend morning updates and report changes in participants baseline status to appropriate staff on a daily basis.

  • Provides nursing care for their assigned participants; in the clinic, at home or over the phone, for triaging and giving nursing advice.

  • Involved in the development and implementation of QAPI activities.

  • Medication management in conjunction with PACE and specialty care providers.  Acts as the backup for the clinic LPN in filling and delivering, medication planners meds discs, medication cubes, and other devices, as needed

  • Providing case management oversight and coordination of plan of care, provides in-home triage and nursing visits as needed.  Plan and coordinate with scheduler for adequate staffing of in-home workers.

  • Maintain accurate, detailed reports and records. Record patients' medical information and vital signs. assessing their conditions and needs in order to develop personalized care plans.  Communicates change in condition or concerns immediately to team

  • Provide on-call coverage during center hours of non-operation.  Be flexible in schedule of hours worked.  Requires use of personal vehicle.

  • Maintain confidentiality regarding participant, staff, contractor, and organizational information.

  • Attend and participate in interdisciplinary team meetings, staff meetings, in-services, projects, and committees as assigned.  





EDUCATION AND EXPERIENCE: 



  • Current RN license in the state of Oklahoma

  • Minimum of two (2) years of demonstrated successful experience; prefer in-home care management experience.

  • Minimum of one (1) year of documented experience working with a frail or elderly population.

  • Experience in the provision of nursing care, including assessment skills, health education, and current nursing treatments and protocols.                                                           



KNOWLEDGE, SKILLS, AND ABILITIES:



  • Working knowledge of physical, mental, spiritual, and social needs of the frail elderly and their families.

  • Effective management skills.

  • Knowledge of utilization review, quality improvement and cost containment systems.

  • Ability to work with ethnically diverse populations.

  • Ability to apply creative problem-solving skills to the complex issues in home care.

  • Ability to work with interdisciplinary teams.

  • Strong organizational skills, flexible, resourceful, and energetic.

  • Effective oral and written communication skills.

  • Good driving record, Oklahoma drivers license and auto insurance required.
