Summary: Monitors the health, safety and well being of participants in the Residential system.   Incumbent is responsible for overseeing and managing the medical and health care of program participants including providing direct nursing care.  Incumbent reports issues to appropriate persons and handles emergencies according to Incompass Human Serivces and other applicable agencies guidelines.  Incumbent must comply with applicable regulatory documentation and standards in the performance of nursing duties. 


Experience & Education:

  • Must be a Licensed Practical Nurse registered with the MA Board of Registration in Nursing

  • At least one year of experience working with the elderly, children, or persons with developmental disabilities.   

  • Strong interpersonal, written, and verbal communication skills required.

  • Demonstrated ability to problem-solve, handle emergencies, and successfully manage several projects simultaneously.

Schedule: M-F / 9:00AM t0 5:00PM

Essential Job Functions:

  • Monitors the health of program participants, including preventative care, and provides professional technical assistance regarding nursing care and medical treatment as needed. 

  • Responsible for management and documentation as it relates to medical information, referrals, reports, medication administration and data collection for use in annual planning, goal setting and intervention strategies.

  • Responsible for the completion of medical record audits and management of medical tracking systems.

  • Assesses individuals as assigned on a regular basis and provides management for individuals with major medical issues.

  • Provides oversight and assistance with the self-medication assessment process.

  • Provides oversight of MAP and for the administration of medical treatments and staff training as needed including annual mandated staff trainings.  Administration is done in accordance with Nurse Practioners Act and DDS and MAP regulations.

  • Provides assistance when needed regarding medication supplies and medical supplies as needed.

  • Orients and trains staff in understanding participants medical needs and in following prescribed programs and plans.   Communicates with and trains staff to complete procedures as appropriate.

  • Provides leadership and support to team during medical crisis.

  • Ensures professional communication with families, DSS, clinicians and others for assigned participants.

  • Provides guidance to staff regarding medical emergencies and informs supervisor of all emergency treatments or 911 calls made.
