Brookside Care Strategies, LLC is currently seeking an experienced MDS Coordinator. Brookside is a 42 bed Behavioral Health Facility located in Muncie, Indiana. Candidates must have either their Indiana LPN or RN License and be in good standing with the State Nursing Board, have excellent organizational skills and experience with Point Click Care is preferred. Position is day shift with flexible start and ending times. Competitive hourly salary based upon years of experience. Position Summary

The MDS Coordinator is responsible for the timely and accurate completion of federal and state required assessments.


Essential Duties and Responsibilities

The essential functions include, but are not limited to the following:




1.       Demonstrates knowledge of RAI, CAA's procedures and the Care planning process.

2.       Demonstrates knowledge and understanding of the Supportive Documentation Guidelines.

3.       Sends required information to reimbursement directors per policy.

4.       Conducts staff in-services relating to MDS issues as needed.

5.      Demonstrates working knowledge of nursing care in accordance with Resident Care Policies and Procedures and is sensitive that the safety and well-being of residents is maintained.

6.       Attends all mandatory in-services.

7.       Maintains and updates Indiana State license in compliance with Indiana regulations.

8.       Acts appropriately under the direction of the CEO and as an active member of the interdisciplinary team.

9.       Communicates and observes the Facility Corporate Compliance Program effectively and complies with Code of Conduct when performing work functions.



1.                 Reviews medical records, resident care plans, consultant recommendations and staff assignments.

2.                 Observes nursing care and visits residents to ensure that nursing care is carried out as directed and treatments and medications are administered in accordance with physician's instructions.

3.                   Completes  the following duties:

a)                 Assigns quarterly assessments to members of the nursing staff.

b)                 Participates in planning and adjustment of Medicare assessment scheduling.

c)                 Validates the entire MDS once printed and ensures documentation is present to support the RUG's N   reimbursable group.

d)                 Writes CAA/CAT summaries assigned to MDS.

e)                 Assists with/teaches care planning process to nurses and other members of the team.

f)                  Prints a Resident Roster and Census weekly.

g)                  Responsible and accountable for the scheduling, accuracy and timeliness of the MDS and Care Plan Process.

h)                 Attends meetings of supervisory and administrative staff.

i)                   Assists with studies related to improvements of nursing care.






1.                 Demonstrates knowledge of each residents historical and diagnostic data in relation to its impact on nursing care.

2.                 Observes and assesses the total resident focusing on all body systems, skin integrity, weight, and hydration status and reports changes in resident condition promptly to the Nursing Supervisor.

3.                 Gathers baseline information to identify potential and acute problems and thoroughly and accurately completes all areas of the MDS nursing admission assessment and quarterly assessment and Medicare assessments using currently approved format.

4.                 Demonstrates knowledge of each resident's therapy schedule and those persons responsible for implementation and management of those services.

5.                 Prepares for and leads team and family conferences in a professional, supportive manner.

6.                  Communicates with nursing and clinical personnel on the implementation of the resident care plan.




1.                  Is aware of, and adheres to, Resident's Bill of Rights and Confidentiality of Resident Information.

2.                  Interacts with residents and family members, co-workers, clinical and ancillary staff in non-judgmental, supportive and calm manner.

3.                  Addresses family satisfaction issues immediately and assists in resolving the matter in a professional manner.

4.                  Is aware of Resident Abuse Reporting Law.

5.                  Participates in the orientation of new staff on ADL and restorative documentation and other documentation as assigned.




1.                  Demonstrates ability to locate emergency equipment (i.e. emergency oxygen tank).

2.                  Understands and demonstrates knowledge of role in Fire and Disaster drills.

3.                  Demonstrates ability to identify and respond appropriately to potential behavioral outbursts.

4.                  Uses facility equipment safely.

5.                  Recognizes, removes, and/or reports potential hazards.




  1. Demonstrates knowledge of and implements Universal Precautions.

  2. Washes hands between resident contacts as appropriate.

  3. Handles linen and disposes of waste properly.

  4. Demonstrates ability to care for residents on isolation precautions.

  5. Demonstrate knowledge of correct positioning/care of catheters and drainage.





1.    Addresses residents in a respectful manner.

2.    Discusses confidential resident information in appropriate areas only.

3.    Knocks on door prior to entering resident rooms, and maintains resident privacy and dignity when providing treatments or other resident care.





Minimum Qualifications (Knowledge, Skills, and Abilities)


1.                 Currently licensed as a Registered Nurse or a Licensed Practical Nurse in the State of Indiana.

2.                  Previous work experience in a long term care setting. Supervisory experience and organizational skills in a long term care setting


3.                 Is listed in good standing as confirmed on the Indiana State Department of Professional Credentialing.

4.                  Possesses excellent technical assessment, documentation skills, and leadership qualities.

5.                  Possesses good communication skills and is patient and self-disciplined.

6.                  Is willing to commit to an interdisciplinary treatment approach to rehabilitation by exercising sound nursing judgment based on preparation, knowledge, skills, understanding and past experience in nursing situations.

7.                 Must be capable of performing the essential functions of the job, with or without reasonable accommodations.

8.                 Must meet health assessment requirements, including two stage Mantoux skin test.

9.                 Must be able to communicate in English, both verbally and in writing, and possess sufficient communication skills to perform the tasks required.




Physical Demands and Work Environment


Occasionally equals approximately 1% to 33% of the workday, Frequently equals approximately 34% to 66% of the work day, and Continually equals approximately 67% to 100% of the work day. (Typical full work day is 7.5 hours)
