JOB STATEMENT


 


The Behaviorist position is responsible for providing behavioral health consultation and therapy to children, adolescents, adults, and families in order to improve psychosocial functioning.


 


JOB FUNCTIONS




  • Conducts interviews, reviews records, administers assessments, and/or confers with other health providers to evaluate mental or physical condition of patients.



  • Works with other health providers to formulate treatment plans.



  • Evaluates crisis situations and determines appropriate interventions.



  • Provides brief, focused interventions in both individual and group settings.



  • Where indicated, works to effect behavioral changes for patients at risk of physical disorders, helps them identify healthier lifestyle choices and develop coping strategies to manage chronic conditions.



  • Monitors, evaluates, and records client progress with respect to treatment goals.



  • Provides parent consultations in regards to childrens treatment progress and parenting support.



  • Coordinates the delivery of behavioral and developmental health care, including linkages with other treatment providers in and out of the primary care setting.



  • Provides training/education to patients, families, and staff on care, prevention, and treatment enhancement techniques.



  • Responsible for completing required documentation of case notes and assessments within a pre-established cycle time.



  • Monitors the clinics behavioral health program; identifying problems related to patient services and making recommendation for improvement.



  • Assists the primary care team in developing care management processes, disease management techniques, case management, and patient education to help improve self-management of chronic disease.



  • Develops strong community relationships and resources with which to support patients in treatment.



  • Participates in Patient Centered Medical Home Care Team collaboration.



  • Performs other tasks as required by management.




MINIMUM QUALIFICATIONS



  • Education: Masters Degree in Social Work, Psychology, Counseling, or related field

  • Specialized Training/Certifications/Licenses:


    • Licensed in the State of Texas (unrestricted) for any of the following:




      • Licensed Clinical Social Worker (LCSW)



      • Licensed Master Social Worker (LMSW)



      • Licensed Professional Counselor (LPC)



      • Licensed Marriage & Family Therapist (LMFT)





    • Current Health Care Provider CPR Certification.




  • Experience:

    • Minimum of 1 yr. working in a primary care setting

    • Minimum of 3 yrs. as a Behavioral Health Therapist



  • Knowledge:

    • Evidenced based treatment models

    • Trauma Informed Care



  • Skills:


    • Basic Computer Literacy


    • Patient-Centered Focus


    • Interpersonal Relations



    • Self-Management



    • Decision Making



    • Innovation/Creativity



    • Bilingual (English and Spanish)





  • Abilities:




    •  Must be able to speak, read, write, and comprehend the English language.



    • Must be able to lift 10 lbs. unassisted





  • Other:




    • Must be able to periodically work some evenings and Saturdays.



    • May require some local travel between different clinic locations.



    • Must be able to maintain required immunizations as required by the organization.
