The LIFE PACE (Program of All-Inclusive Care for the Elderly) Clinic team performs a vital service by providing primary care for Tulsa's aging population.  The goal of our amazing team of professionals is to provide outstanding services and oversight in order to keep participants in their homes for as long as safely possible in lieu of nursing facility placement. 



PRIMARY CARE RN

(Monday through Friday position, day shifts only, no holidays or weekends)




We are looking for a Registered Nurse to grow our staff and join a team of professionals, who excel in providing unique health care for Seniors! The Program of All-Inclusive Care for the Elderly (PACE) offers older adults and their caregivers a welcome alternative to nursing home care. Participants have access to comprehensive medical services and social support at the site while they enjoy independent living in their own homes and communities.



Primary Care RN is a hands-on clinic position, with responsibility to provide case management alongside the delivery of direct nursing care to a specific panel of PACE participants. The ideal candidate will be able to give the best nursing care in an outpatient, clinic setting, following standard health and safety guidelines. Since this candidate will be working exclusively with seniors, some with dementia, patience and extra compassion will sometimes be required.  



As a Registered Nurse, you have an essential role in conducting patient visits to assess their nursing needs. During a participants initial clinic visit, the RN works alongside the physician or NP in order to develop the plan of care. With each successive visit with your participants, you will perform ongoing comprehensive assessments of their needs while making any necessary revisions to the plan of care under the direction of the physician or NP. Using your specialized nursing skills, you provide the services or treatments needed in the clinic as well as educate the patient or caregiver regarding the disease process, self-care techniques, and prevention strategies.  Wound care experience is preferred.  In addition, through direct communication with physicians and other staff members, you ensure that all necessary parties are kept informed about the condition and needs of each participant in your care. You re-evaluate the nursing needs of participants and assist with the discharge procedures when they have been in the hospital or ER. 




The clinic primary care RNs share on call responsibilities with the Clinic Manager and Clinic LPN, which consists of handling issues which arise after-hours and on weekends. Call is one-week in duration, and frequency depends on the number of staff; currently on call will be 1 in 8 with this position filled. The physicians/NPs are also on a call rotation with the nurses, so you have coverage as needed.  As an additional bonus, and to say thank you to the nurse on call, the Friday following your call week, you are allowed to leave at noon without using PTO.

EDUCATION AND EXPERIENCE: 



  •         Current RN license in the state of Oklahoma

  •         Minimum of two (2) years of demonstrated clinical nursing experience; prefer wound care experience.

  •         Minimum of one (1) year of documented experience working with an elderly population.

This is a Safety Sensitive position requiring candidates to have:



  •         A background check,
