Speech Language Pathologist - CFY

Location: Houston



Circle of Care is a pediatric home healthcare company providing therapy services throughout Texas. We are currently seeking a full-time salaried position for a Speech Language Pathologist - CFY in our Houston service area, treating patients (age birth through 20) in the home, daycare or community setting as prescribed by the attending physician.



We utilize an excellent Electronic Medical Records system for documentation, as well as admission paperwork. The corporate office offers 100% support for our therapists, whether it is scheduling, contacting parents, etc. The therapist can be completely autonomous or rely upon the office for support - you make the call. Our Administrator is a treating therapist, so the mindset of the agency is to make every effort to provide the needed support to ensure the therapists are able to provide the best possible care for our patients...patient care is very important and our therapists are extremely respected and appreciated. 


 


Job Summary:


The Speech Pathologist/Audiologist CFY is a qualified professional who evaluates and provides speech therapy services to patients in the home as prescribed by the attending physician. 




Qualifications:



  • Certified member of the American Speech and Hearing Association

  • At least a Master's degree 

  • Licensure or certification by the state of Texas as an SLP



 



Responsibilities:



  1. Provide assessments and treatment activities with patients within the parameters of the regulations outlined by ASHA regarding the Clinical Fellowship Year (CFY) with the appropriate supervision of a licensed Speech-Language Pathologist

  2. Schedule and plan activities to provides rehabilitative therapy for speech and language disorders

  3. Records type of treatment and patients reaction to it on clinical/progress notes, which are written on the day of the visit and incorporated in the patients record weekly

  4. Maintains adequate records on all patients, including summary reports

  5. Utilizes community resources and Agency personnel for proper referral

  6. Selects and administers diagnostic and therapeutic techniques and materials

  7. Instructs and counsels other health team personnel and family members in methods of assisting patient in improving, correcting, and accepting his/her disabilities

  8. Attends case conferences as requested

  9. Participates in staff development activities and in-service education as requested

  10. Confirms, on a weekly basis, the scheduling of visits with the Patient Care Coordinator to coordinate necessary visits with other personnel

  11. Notifies Agency of absences due to illness, emergency leave, normal vacation periods, or special professional meetings, which will all affect agreed services with the Agency

  12. Participates with staff, patient, and physician in discharge planning activities and completes the speech therapy discharge summary within the designated framework of the Agency policies


Special Requirements:



  • Reliable transportation

  • Must have a car with current insurance coverage; and

  • Valid state drivers license
